Passport for Good Health

The Passport for Good Health program provides booklets for recording immunization and health information. There is a newborn and children's edition as well as a senior edition. The committee distributed 10,836 passports from June 2010 through May 2011.

The Passport for Good Health program provides immunization records (passports) to nine local agencies:

  • Conroe Regional Medical Center
  • Herrin Pediatric Clinic
  • Memorial Hermann - The Woodlands Hospital
  • Norwood Pediatric Clinic
  • St. Luke’s Community Medical Center
  • Tamina Community Center
  • The Friendship Center
  • Willis Parent's Resource Center
  • Montgomery County Public Health

The Passport for Good Health program also provides newborn layettes to Conroe Regional Medical Center for economically disadvantaged parents. Over 45 layettes are provided each month.  In addition , a group of volunteers hand knits hats for distribution to newborns at a local hospital.

A Senior Edition booklet was also developed so that recipients could record their medical history, medications, family contacts, and health-care guidelines. These passports are distributed through local senior organizations.
